Admission Documentation Form

If you have signed up for an EMT course, please submit the following documents before the course begins:

​

1. Valid ID: A copy of your valid identification.

2. Educational Proof: A copy of your high school diploma, GED equivalent, college transcript, or proof of current year high school completion.

3. BLS CPR Certificate: A copy of your current Basic Life Support (BLS) CPR certificate.

4. Immunization Records: Complete set of immunization records as listed below:

   • TB (Tuberculosis):

     • Negative TB test within the last 12 months and valid through the end of your chosen course

   • MMR (Measles, Mumps, Rubella)

   • Tetanus: Within the last 10 years.

   • Varicella (Chicken Pox)

   • Hepatitis B

 

You may send these documents as you have them available. Please ensure all documents are submitted promptly to avoid any delays in your course start.
